ACL, in partnership with the CDC, is pleased to announce the selection of 58 organizations for participation in the Community Care Hub National Learning Community (NLC). The organizations, representing 32 states, will engage in one of two learning tracks to bring together organizations serving as Community Care Hubs that are either in development or interested in expansion.

ACL is proud to announce the final winners of the Social Care Referrals Challenge. Launched in March 2020, the competition sought IT solutions to support health care systems and community-based organizations in partnering to provide holistic health and social care for older adults and people with disabilities. 

The Grandfamilies & Kinship Support Network is identifying and designating exemplary kinship and grandfamilies policies, practices, and programs, with the goal of sharing replicable strategies with others. The Network encourages applications from government agencies in states, tribes, and territories, as well as kinship navigator programs and other community-based organizations that serve kinship and grandfamilies.
