ACL is pleased to announce the 2020 cohorts of Chronic Disease Self-Management Education (CDSME) and Falls Prevention grantees. The project period for these grants is August 1, 2020 – July 31, 2023. The new CDSME grantees will reach an estimated 21,000 participants through evidence-based CDSME and self-management support programs. The cooperative agreements are intended to increase the number of CDSME program participants, while concurrently enhancing the sustainability of these proven programs.
