COVID-19 has disproportionately impacted people with chronic conditions. Given this, it is important for health coaches to provide continued self-management support for both COVID-19 and other health concerns. In fact, health coaches can reduce chronic disease burden and address social determinants of health during the COVID-19 pandemic.

However, the pandemic has impacted delivery of health coaching and wellness planning. This new tip sheet from RIC highlights promising practices on administering health coaching and wellness planning during COVID-19 for providers and health plans that serve individuals with chronic health conditions. It also provides tips for addressing COVID-19-related concerns and overcoming virtual barriers.
