Is your organization operating or planning to become a community care hub? The Center of Excellence to Align Health and Social Care’s 2025 Community Care Hub National Learning Community (NLC) is offering a unique opportunity to strengthen your organization’s capacity, expand existing partnerships, and share your insights with peers nationwide.
Running from January through August 2025, the NLC will provide:
- Tools to build operational readiness and craft value propositions for health care partners.
- An online platform for resource sharing and discussion.
- Access to subject matter experts and one-on-one consultation.
- Monthly peer learning sessions and multiple “office hours” opportunities.
About Community Care Hubs
A community care hub (CCH) is a community-focused entity that organizes and supports a network of community-based organizations providing services to address health-related social needs. A CCH centralizes administrative functions and operational infrastructure, including but not limited to contracting with health care organizations; payment operations; management of referrals; service delivery fidelity and compliance; and technology, information security, data collection, and reporting. A CCH has trusted relationships with and fosters cross-sector collaborations of local community-based and health care organizations.
