ACL’s No Wrong Door Resource Center invites you to participate in an upcoming webinar, "Seamless Transitions: Supporting Patients With Chronic Illness Across Care Settings." This webinar will focus on:

  • Critical intervention points where community-based organizations can effectively support people with chronic conditions during hospital discharge planning, transition back to home, and post discharge
  • Long-term supports to improve community tenure and avoid hospital readmissions

In No Wrong Door systems, state agencies retain responsibility for their respective services but coordinate to integrate access to services available through other agencies through a single, standardized process. By helping people connect to the services they need to live in the community, these systems help divert them from more costly forms of care, such as nursing homes, as well as help them avoid unnecessary hospital admissions and readmissions.
