Congratulations to Administration for Community Living (ACL) grantee MAC Inc., in partnership with Peninsula Regional Medical Center (PRMC), for receiving the Population Health Innovator of the Year award!
The award was presented by Health Quality Innovators (Maryland’s Quality Innovation Network-Quality Improvement Organization) and recognizes MAC Inc. and PRMC’s collaboration on Project Living Well, which is an innovative initiative that focuses on expanding access to evidence-based self-management programs on Maryland’s lower Eastern Shore (particularly in communities with higher chronic disease prevalence and other health disparities).
