Partnership to Align Social Care Webinar on Exploring Core Functions of Community Care Hubs
A Look at Network Recruitment, Engagement, and Support and Operations

Building the capacity of Community Care Hubs (CCHs) is an important component of ACL’s vision for coordinated, person-centered care supports for older adults and people with disabilities. ACL is supporting CCHs through a National Learning Community, as well as through community infrastructure grants and a recently posted Notice of Funding Opportunity to support a Center of Excellence to Align Health and Social Care.
Among health systems, health care providers, and health insurers, the importance of addressing social determinants of health — particularly for those with complex care needs — has come into sharp focus. The Partnership to Align Social Care is a cross-sector effort to co-design a multi-faceted strategy to enable successful partnerships between health care organizations and networks of community-based organizations (CBOs) and CCHs.
Join the Partnership and health and social care sector leaders for the final webinar in a three-part series offering a deep dive into the roles and responsibilities of advanced CCHs improving alignment between health and social care services.
This final webinar will explain the core components of CCH network recruitment, engagement, and support, as well as operations activities.
